Walk Registration
Registration for the 2010 Walk has begun!
Thank you for registering and Making a Difference! Please consider increasing our fundraising efforts by having friends, family members, classmates, coworkers and local businesses sponsor you for the walk. Walker Sponsor Forms can be obtained upon request.
Registration and Payment is easy!
- Please enter your information on the registration form below and ensure that you include the number and size of of a T-Shirt for each participant. Then...
- Proceed to make Payment via the submit button.Use Pay-Pal or Pay by credit card-Look for..... "Use your credit card or bank account(Where available). Continue"
- Print the form if you are mailing your payment.
- Additional Donations & Walk Sponsorships can be made at registration or on the donations page & the on line store page.
- Please note in the comments section special verbiage for mile sponsor signs your Family name or in "Honor of" or "Memory of" someone.
Payment Methods-Credit Card and PayPal payments are preferred for efficiency.
Mail payments/checks: Please print a copy of the registration page and mail a completed copy with your Check to 326 Madison Road, Springfield Pa 19064. Checks can be made payable to either "PMP Cancer Walk" or the "PMP Research Foundation"
